Our evaluation of possible food allergy starts with an initial consultation to obtain a detailed history of the allergic reactions that have occurred. This is followed by skin testing to diagnose and or confirm a particular food allergy. We use a painless, needle-free skin test that is well-tolerated by young children. The test is quick to apply, even in infants and gives an immediate result which the doctor will interpret for you. Sometimes, a blood test will also be necessary. The final result will be a complete allergy profile of the patient.
Oral challenges are performed in the doctor's office and are usually done when the medical history and allergy tests, such as skin and blood tests, are inconclusive. The oral challenge is a more definitive test because it will show whether the food ingested produces no symptoms or triggers a reaction.
One situation is when a child may be outgrowing their milk or egg allergy but testing cannot predict whether the food will be tolerated. Another example is when a child has had blood testing done by their primary doctor that is "positive" for a particular food, but the results are weakly positive and not clearly anaphylactic.
The particular food is given to the child in the office with a series of doses over the period of several hours. Passing an oral challenge means the child is not allergic, and the food does not have to be avoided.
